Long Term Kratom Use

Most people who use kratom regularly reach a point where they wonder what years of it look like. It’s a fair question. It’s also one the research can’t answer yet.

This page covers what the studies measured, how long they followed people, and where the gaps sit. It does not tell you how long is too long. Nobody can know that number. What you can do is learn what the evidence covers, and then watch your own pattern more closely than you watch anyone else’s.

What Does Long Term Mean in Kratom Research?

Less than you would hope, and the phrase is used loosely.

Some studies call six months long term. Others mean twenty years. Findings do not carry from one to the other. A study of people who have used kratom for two decades describes something very different from a study that gave volunteers kratom for fifteen days.

Both of those exist. Neither one followed the same people forward through time.

How Long Has Kratom Actually Been Studied in a Controlled Trial?

Fifteen days.

The largest controlled kratom study run so far was published in January 2026. Its authors describe it that way themselves. It involved 116 healthy volunteers. Forty nine were given dried kratom leaf powder and 67 were given a placebo.

The design was strong. It was randomized, double blind and placebo controlled. The whole study ran 47 days, and that covered one round of single servings, fifteen days of daily use, and a follow up period.

Fifteen days of daily use is what that gets you. What the study found is real. None of it describes year three or year ten.

That trial belongs on the record with its funding attached. It was paid for by the company behind the product tested. Two of the authors were paid by that company for advice on how the study was designed, and every author was paid for analyzing the data and writing it up. They say so in the paper. Read the result as what a well run company funded trial found, which is not nothing and is not independent either.

The volunteers were also screened hard before they started. They were nonsmokers, ages 18 to 55, and either new to kratom or free of it for at least twelve months. Anyone with certain known genetic differences in how the liver processes drugs was excluded. That’s not the same group as a person who has taken kratom every day for six years.

The study did find that side effects rose as the amount rose. After the fifteen days, the most commonly reported were headache, feeling hot, nausea, and raised liver enzyme readings.

That last one is worth sitting with. A liver marker moved inside two weeks, in volunteers who were screened healthy before they started. Nobody has looked at what the same marker does over years, in people who were not screened for anything.

What Do the Longer Studies Actually Show?

They show a moment, not a course.

Past fifteen days, every study is a snapshot. Researchers find people who already use kratom, measure them once, and compare them against people who do not.

The largest of these was published in January 2026. It studied a community in Southern Thailand where kratom use is traditional and locally permitted. It compared 285 users against 296 non-users. Everyone came from the same community, and the groups were matched for age.

That study declares no competing interests, and it was paid for by Thai public research bodies and a Thai university. Worth saying, because the other two studies on this page cannot say the same.

The most useful thing in that paper is not a finding about kratom. It’s a finding about how easily this subject gets read wrong.

At first the users’ blood results did look different. White cell counts, red cell counts, platelet counts, and several other measures. Then the researchers adjusted for the other ways the two groups differed. The users were more often male. They weighed less. They smoked and drank far more often. 54 percent of the users smoked, against 13 percent of the non-users.

After that adjustment, every one of those blood differences was gone. So was every difference in the liver markers.

The lesson generalizes. When you read that a group of kratom users had some result, ask what else was true about that group. In this case the answer was most of it.

The authors close by saying the study that would settle any of this has not been run. What’s needed is one that follows the same people forward over years. It does not exist.

What Is Known About People Who Have Used Kratom for Decades?

Very little, and it rests on thirteen men.

A Malaysian study examined thirteen men who had used kratom for more than twenty years. There was no comparison group at all. Twelve of the thirteen were smokers, and had been for more than twenty five years on average.

The researchers found cholesterol and inflammation markers above the normal range. They also said plainly that smoking, diet and lifestyle could account for that rather than kratom.

Thirteen men, measured once. That is the deepest published look at decades of kratom use. It’s not enough to build a rule on, and the researchers did not try to.

Does Centuries of Traditional Use Answer the Question?

No, and this is the argument you will hear most often.

Kratom has been used in Southeast Asia for centuries. That’s true. It gets offered as reassurance constantly. Look at what those people are consuming before you accept it.

A 2025 study interviewed 29 daily kratom users in rural Malaysia. Their average age was 61. They had used kratom for an average of 22.3 years. Most drank a tea brewed from leaves harvested fresh. More than half chewed fresh leaves. Some ate young shoots raw, mixed with chili or coconut. Every one of them grew their own trees on their own land.

That paper carries disclosed interests and they belong here rather than in a footnote. One author founded a consultancy that works with the kratom industry. Another is executive director of a kratom industry coalition. The research was paid for by an industry alliance. The lead author declares nothing. Worth noting which way it points, because the finding takes away the reassurance argument this industry leans on hardest.

The researchers state it themselves. Fresh kratom is not available in Western markets, and Western products are dried leaf powder or extracts made from that powder.

That’s not what we sell either. Whole Earth Gifts sells dried powder, capsules made from that powder, and extracts concentrated from it. Not one of them is the form of kratom in those studies, and no US seller offers the form that is.

So the long human record does exist. It’s a record of a different form of the plant, prepared a different way, by people whose daily lives look nothing like yours. It describes them. It does not describe you.

What Is Regular Kratom Use Understood to Lead To?

None of these sit at the rare edges of regular use. They are the ordinary direction regular use travels. Understanding that direction is worth more to you than any figure a study could hand you, because the direction has been documented repeatedly and the figures have not held up.

How Would You Know Where Your Own Use Stands?

By having written it down before the question came up.

No study can tell you where your ceiling sits. Your own record is the better instrument, and it’s the only one built from your body rather than someone else’s.

Worth tracking:

  • Whether the amount you take has moved, and how fast
  • Whether you have gone a full day without it, and how that day went
  • Whether the reason you reach for it has changed
  • Whether you have tried to cut back and could not
  • Constipation, sleep, appetite and skin

Memory is a poor tool for this. A written record from six months ago will disagree with what you remember, and the record will be right.

When Should You Involve a Physician?

Before it becomes urgent, and certainly before you stop after a long stretch of regular use.

Speak with a physician if:

  • You have any liver or kidney condition
  • You take prescription medication of any kind
  • You notice yellow skin or eyes, dark urine, or stomach trouble that does not settle
  • You have tried to stop and could not
  • You are pregnant or nursing

Tell them you use kratom. Say how much, and say how long. They cannot help you with half the picture, and kratom does not show up on a standard drug panel.

What Has Not Been Answered?

Nearly all of it.

Nobody has followed a group of kratom users forward through years and measured what happened. Every study on this page is either short or a single snapshot. Researchers who work on kratom say this in their own papers, and they say it as the main limitation rather than a footnote.

That gap is the reason this page publishes no ceiling on duration. Not because a rule prevents it. Because nobody knows it. Anyone who hands you a number for how long is safe is working ahead of the evidence, and so is anyone who hands you a number for how long is too long.
